We are invested in the well-being of you and your child(ren) and would like to share the THRIVE! Consent Form with you. This form is available in English, Haitian, and Spanish to meet the needs of all children and families in our community. If you would like to participate in the THRIVE! Program, please complete the form and return it to school in your child’s backpack.

Please note that only one form is needed for all the children in your family. And remember that your information is always confidential and is only shared between you and the PHA school administration.

PHA THRIVE is currently available for students in Kindergarten through the 6th grade.
